Always Events; Phase 4: Scale-up and spread
NHS England, in collaboration with the Institute for Healthcare Improvement (IHI) and Picker have developed a programme to implement Always Events within the NHS in England. This report evaluates the initial pilot programme, covering trusts involved in phases 1 and 2.
